CPCA Medi-Cal Health Navigator Project Kicks Off January 2024
CPCA officially launched the Medi-Cal Health Navigator Project in January 2024, which is providing $17.75 million to 108 community health centers (CHCs) and 10 consortia to meet California’s Medi-Cal redetermination and expansion goals through June 2025. Activities across the state are underway and CHCs are providing a range of navigation services to their patients, including Medi-Cal outreach, enrollment, renewal, utilization, and troubleshooting. .
CPCA is administering these funds as a Third-Party Administrator in conjunction with the Department of Health Care Services (DHCS). As part of the project launch, CPCA hosted a series of trainings and office hours over several months to equip primary recipients and subrecipients with the necessary information to meet grant requirements, including monthly and quarterly reporting. In addition, CPCA compiled numerous resources – guides, Frequently Asked Questions, checklists, monthly newsletters, and more – in a Resource Library that provides grantees with a centralized location for all important information related to the project.
CPCA also partnered with Azara Healthcare to provide a robust data reporting system, Azara Care Connect (ACC), that collects aggregate and patient-level data in a secure and HIPAA-compliant manner. Azara Care Connect provides multiple avenues for participants to submit their monthly data, which meets the diverse data and technology needs of small and large CHCs. Azara’s partnership, expertise, and ACC technology have played an invaluable and critical role in meeting the state’s data reporting requirements. Azara is actively collecting data for the first quarter, which will provide an initial glance at the impact of this project.
